This Privacy Policy explains how Bethany Home Discount Pharmacy LLC (“we,” “us,” or “our”), 6601 W Bethany Home Rd, Ste A10-11, Glendale, AZ 85301, collects, uses and shares information through athomevaccines.com (“At Home Vaccines”), including the home-visit request form. At Home Vaccines is a service provided by Bethany Home Discount Pharmacy.

1. Your health information and HIPAA

As a pharmacy, we are a covered entity under the Health Insurance Portability and Accountability Act (HIPAA). Once you become our patient, the health information we create or receive while caring for you is protected by HIPAA. That use is described in our Notice of Privacy Practices, which you can get at the pharmacy or by calling 623-440-7717. 2. Information we collect

Information you give us in the visit request form

We only ask for what we need to call you back and plan a home visit:

Who the request is for (yourself, someone you care for, or a facility), and your name if you’re requesting for someone else
The patient’s full name and date of birth
The patient’s gender and race or ethnicity, only if you choose to share them. These are optional, help complete vaccination records, and never affect care.
A phone number and an email address
Your preferred language
The vaccines you’re interested in (or “not sure”) and how many people at the address need vaccines
The visit address, city and ZIP code, which we use to plan the visit and figure out the service fee based on distance
Preferred days and time of day
A record that you checked the consent boxes, with the date and time

We do not ask for Social Security numbers, insurance member numbers, payment card details or medical history through this website. We collect what we need for your care, such as insurance and screening questions, by phone or in person.

Information collected automatically

No analytics, advertising or tracking tools. We don’t use Google Analytics, advertising pixels, social media trackers or similar technologies.
Server logs. Like most websites, our web hosting provider may automatically record basic technical information such as your IP address, browser type and the pages requested, for security and to keep the site working.
On-device storage. We save your language setting and privacy choices in your browser. See our Cookie Policy.

3. How we use information

To respond to your request and contact you by phone or email about scheduling a home visit
To confirm your service fee, check insurance coverage and plan the visit
To check which vaccines the patient may be eligible for and to complete vaccination records
To provide pharmacy services, bill insurance and keep records the law requires, once you become a patient
To keep our website secure and prevent spam or misuse
To comply with the law

We do not use form information for marketing without your separate permission. We do not sell your personal information or share it for targeted advertising.

We share information only as described here:

Form delivery. When you submit the request form, your browser sends it to FormSubmit (formsubmit.co), a form-delivery service. FormSubmit emails it to our pharmacy inbox. FormSubmit’s handling is described in its own privacy policy.
Email hosting. Our pharmacy email is provided by Google (Gmail), so submitted requests are stored in that account.
Website hosting. Our hosting provider processes technical data to deliver the website.
Google Maps (only if you choose). The map on our site stays off until you choose to load it. If you do, Google receives information such as your IP address and may set cookies.
Care and payment. After you become a patient, we share information with insurance plans, healthcare providers, public health registries and others as HIPAA allows or requires. This is described in our Notice of Privacy Practices.
Legal reasons. We may share information when the law requires it, to respond to lawful requests, or to protect the safety of our patients, staff or the public.

5. How we protect information

We use reasonable safeguards to protect information, including an encrypted (HTTPS) connection for this website, and we limit who at our pharmacy can see requests. No website or email system is completely secure, though. If you’d rather not send details online, please call us at 623-440-7717 instead.

6. How long we keep information

We keep visit requests only as long as we need them to contact you and schedule your visit. If you become a patient, we keep your records for as long as pharmacy and healthcare record laws require. We delete requests that don’t lead to a visit once we no longer need them.

7. Your choices and rights

Ask us about your information. You can ask to see, correct or delete the information you sent through this website. Call 623-440-7717 or email bethanyhomepharmacy@gmail.com. We may need to confirm who you are, and we may keep information the law requires us to keep.
HIPAA rights. As a patient, you have rights over your health information, including the right to get a copy of it. Our Notice of Privacy Practices explains these rights.
Privacy choices. You can turn optional Google Maps content on or off at any time with the “Privacy choices” link at the bottom of every page.
Global Privacy Control. We don’t sell or share personal information for advertising, so there’s nothing to opt out of. We still respect browser privacy signals.

8. Requests made for someone else

If you submit a request for another person, you confirm that you have their permission or are legally allowed to act for them. We’ll confirm the details directly with the patient or their representative before any vaccine is given.

9. Children

This website is not directed to children under 13, and we don’t knowingly collect personal information from children through it.

10. Changes to this policy

We may update this policy from time to time. We’ll change the “last updated” date above, and the updated version will apply from that date.
