These Terms & Conditions (“Terms”) apply to your use of this website and to home-visit vaccination requests made through it. The website athomevaccines.com and the At Home Vaccines service are provided by Bethany Home Discount Pharmacy LLC (“we,” “us,” or “our”). By using this website or submitting a request, you agree to these Terms. If you don’t agree, please don’t use the website. You can still call us at 623-440-7717.
1. Not for emergencies, and not medical advice
If you are having a medical emergency, call 911. Nothing on this website is medical advice. Vaccine information here is general, based on public health guidance that can change, and doesn’t replace advice from your doctor or pharmacist about your own situation.
2. Visit requests are not appointments
Submitting the request form does not book an appointment or guarantee a visit. A visit is scheduled only after a member of our pharmacy contacts you and confirms the date, time, service fee and insurance details. Whether we can visit depends on things like location, pharmacist availability and vaccine supply.
3. Service area
We offer home visits to addresses within about 15 miles of our pharmacy at 6601 W Bethany Home Rd, Glendale, AZ 85301. Any city or town names on this website are examples only. Some parts of those areas may be outside our service area. If you live farther away, call us to ask about options.
4. Service fees
Every home visit has a service fee, which is separate from the cost of the vaccine. The fee depends on the distance from our pharmacy to the visit address. Current fees are listed in the answer to “How much does a home visit cost?” on our home page. Some visits, such as group home visits where more than 5 residents are vaccinated, have no service fee, as described there. We’ll confirm your fee by phone before your visit is scheduled. Fees may change, but the fee we confirm with you when scheduling is the fee that applies to that visit.
5. Vaccines and insurance
We bill the cost of your vaccine to your insurance plan when your plan covers it. Coverage, copays and deductibles depend on your plan. You are responsible for any vaccine costs your plan doesn’t cover. We’ll discuss any expected out-of-pocket vaccine costs with you before your visit. Insurance approval is not guaranteed.
6. Pharmacist review and consent
Before any vaccine is given, our pharmacist will review screening questions and a consent form with the patient or their legal representative. The pharmacist may decide not to give a vaccine if it isn’t appropriate or safe, or if the conditions at the visit don’t allow it.
7. Your responsibilities
- Give accurate, complete information and let us know if it changes.
- Only submit a request for someone else if you have their permission or are legally allowed to act for them.
- Call us as soon as you can if you need to reschedule or cancel.
- Help provide a safe place for the visit.
- Don’t misuse the website, for example by sending spam, false requests or harmful code.
8. Website content
We try to keep this website accurate and up to date, but information such as available vaccines, fees, hours and service areas may change. If this website and what our pharmacy tells you directly disagree, what our pharmacy confirms with you applies. The website content, including text, logos and photos, belongs to us or is used with permission and may not be copied without permission. 9. Third-party services and links
This website uses third-party services, including a form-delivery service and an optional Google Maps embed, and links to other websites. We don’t control those services or websites and aren’t responsible for their content or practices. See our Privacy Policy and Cookie Policy.
10. Disclaimers and limitation of liability
This website is provided “as is” and “as available.” To the fullest extent allowed by law, we disclaim all warranties about the website, including that it will always be available or free of errors. To the fullest extent allowed by law, we are not liable for indirect, incidental or consequential damages that come from using the website. Nothing in these Terms limits any liability that can’t be limited under applicable law, or your rights as a patient.
11. Accessibility
We want everyone to be able to use this website, and we work to follow the Web Content Accessibility Guidelines (WCAG) 2.1, Level AA. If something on this site is hard for you to use, please call 623-440-7717 or email bethanyhomepharmacy@gmail.com. We’ll help you another way and work to fix the problem.
12. Governing law
These Terms are governed by the laws of the State of Arizona, without regard to its conflict-of-law rules.
13. Changes to these Terms
We may update these Terms from time to time. When we do, we’ll change the “last updated” date above. If you keep using the website after an update, the updated Terms apply.
